A Consideration for Treatment of Right Colonic Deverticular Disease to be Misdiagnosed as Acute Appendititis


Abstract
Of colonic diverticulosis, diverticulosis of the cecum is unusual, Therefore, many surgeons will encounter the unexpected right colonic diverticular disease during appendectomy and must choose the appropriate treatment once they make the
diagnosis.
Twenty-two cases of right colonic diverticular disease were treated at the Dae-Dong Hospital from January, 1993 to June, 1995. The ratio of male to female to female was 1.4:1. Peak incidence was the fourth decade of life. All of the patients had
right
lower quadrant tenderness with or without rebound tenderness and had a preoperative diagnosis of acute appendicitis In all twenty-two cases, diverticula were found in the operative field during surgery. All of the cases were confirmed to be
diver-ticular disease by pathologic and radiologic study postoperatively. Resectional therapy was performed in seventeen cases, and conservative treatment in five cases. Diverticulectomy was performed in eleven cases, partial cecectomy in four
cases,
cecectomy in one case, and right hemicolectomy in one case, respectively. Appendectomy was universally performed in all twnety-two cases. With one exception, all operations were performed curatively and successfully; there were no significant
complications during follow-up studies. In conclusion, a safe and effective treatment for cecal diverticular disease is surgical removal of diverticula alone by means of resectional modalities including partial cecectomy or wedge resection,
whichever is
more feasible.
